Job Description

Position Summary:
The Program Associate will play a key role in supporting the delivery of community-based
programs focused on HIV prevention, education, and wellness. This position is responsible for
providing HIV testing and counseling services, facilitating program groups, conducting
community outreach, and linking individuals to appropriate healthcare and supportive services.
The ideal candidate is compassionate, community-oriented, and committed to advancing health
equity for diverse populations.
Key Responsibilities:
• HIV Testing & Counseling – Duties/Responsibilities
o Conduct targeted community outreach, risk reduction education, HIV/HCV rapid
testing, referrals, and appropriate case documentation with follow-ups based on
client needs.
o Complete comprehensive assessments to identify client needs for medical
treatment, prevention (PrEP/PEP), counseling, and supportive services, tailored to
their health literacy and language preferences.
o Provide risk reduction sessions on- and off-site, deliver HIV/HCV education, and
make referrals for additional supportive services (e.g., food banks, transportation,
mental health, community forums).
o Maintain health screenings and testing in a HIPAA-compliant environment, and
identify new partners or hotspots for HIV rapid testing.
o Collaborate with medical providers and multidisciplinary teams (Linkage to Care
Specialists, PrEP/PEP Navigators, Case Managers) to ensure streamlined linkage
to care and confirm service outcomes.
o Assist clients with navigating healthcare systems, including insurance coverage,
medication access, and patient assistance programs to reduce barriers to care.
o Complete accurate data collection, ensure consistent follow-ups, prepare monthly
reports, attend program meetings, and perform other duties as assigned.
• Program Facilitation
o Facilitate group sessions and workshops that address HIV prevention, health
education, and wellness.
o Create a safe and affirming space for participants to engage in discussions around
sexual health, identity, and community issues.

o Assist in the development of group curricula and materials as needed.
• Outreach & Engagement
o Conduct targeted outreach in community settings, both in-person and online, to
recruit participants for programs and services.
o Conduct outreach calls to current and prospective participants to promote services
and encourage engagement.
o Build relationships with community stakeholders and partner organizations to
increase visibility and impact.
o Distribute educational materials and promote services at community events,
venues, and health fairs.
• Program Support & Administration
o Maintain accurate and timely documentation of testing, counseling, group
activities, and outreach encounters.
o Assist with program evaluation by collecting participant feedback and
contributing to reporting requirements.
o Support the planning and coordination of community events and program
initiatives.
o Perform other duties as assigned.

Qualifications

• High School Diploma (bachelor’s degree preferred in Public Health, Social Work,
Psychology, or related field; or equivalent experience in community health programs.)
• HIV test counselor certification through the Division of HIV and STD Programs (DHSP)
(or willingness to complete training within a specified timeframe).
• Experience providing HIV/STI counseling, testing, or case management preferred.
• Strong group facilitation and public speaking skills.
• Experience with community outreach, engagement, and relationship-building.
• Commitment to serving LGBTQ+ communities, communities of color, and individuals
impacted by HIV.
• Strong organizational skills with the ability to manage multiple responsibilities.
• Proficiency in Microsoft Office and ability to learn data entry systems.
