A List Therapy Group is seeking a Physical Therapist to treat patients in the Home Health setting in multiple location through-out Southern California. A List Therapy Group is Therapist owned Home Health Agency that provides exceptional In-Home healthcare services to patients of all ages. We connect patients with our experienced and compassionate clinicians.

Physical Therapist (PT) Job Description/Requirements:
• Evaluation/Treatment of Patients: Complete evaluation and treatment of patient in compliance with agency policy, law and regulation and within the Physical Therapy Professional Scope of Practice.
• Assist in evaluating level of function and in developing the plan of care. Evaluate the home environment and makes appropriate recommendations
• Provides Continuity of Care: Ensure appropriate planning for patient discharge/reassessment and referral to other services according to agency policy. Advise, consult and, when appropriate, instruct family and other agency personnel in patient’s therapy program and needs.
• Communicates: Communicate with care coordinator and other health care professionals caring for the patient to ensure uninterrupted patient care.
• Observe records and report information on the patient’s condition to the attending physician and in the patient’s health record.
• Supervise Physical Therapist Assistants when assigned
• This is an independent contractor position and the pay is $80-120 per visit. visits are 45 minutes in length.
• Locations: Los Angeles County, Orange Count, San Bernardino County, Riverside County

Minimum Required Qualifications:
• Current CA Physical Therapy (PT) License
• Valid California Driver License/ with transportation insurance
• EIN/ with Business License required
• Mal-Practice liability Insurance
• Physical within 2 years, negative TB test within 1 year
• CPR
