LCSW, MSW or ASW

You will meet with patients and their families to assess, develop and recommend a care plan. Collaborate with our staff Physicians and medical team to manage and document the patient's progress. Make recommendations for kidney transplant. Assist with transportation, hospital admission, and discharges. Be familiar with available community resources to refer patients and their families. The role of our Social Worker's are an integral part of the care team. We offer a challenging and rewarding opportunity.

LCSW, MSW or ASW with previous clinical experience. ASW's will be overseen by a LCSW, that will be available to validate ASW's hours.
Must be compassionate, dependable, ethical and an excellent communicator.
