The Registered Associate Clinical Social Worker (Registered Associate CSW) - Healthy Steps Specialist (HSS) is a member of the interdisciplinary health care team, which functions to meet the objectives of the community medical center where they work. The Registered Associate CSW - HSS will provide services billable under the FQHC Guidelines. The Registered Associate CSW - HSS is knowledgeable regarding Birth-5, will provide a mental health evaluation, provide brief interventions, crisis management, community referrals and follow-ups as appropriate. The Registered Associate CSW - HSS will work toward building alliance with both internal and external partners to strengthen the family’s well-being and will be under the supervision of the Director of Behavioral Health Services and Behavioral Health Clinical Supervisor.
1. Provides and completes an initial behavioral health evaluation to patients referred to Behavioral Health Services through the Healthy Steps Program during the first initial session.
2. Develops treatment goals and objectives with the patients/families to address behavioral health, psycho-social, emotional, and other needs by the second appointment. Services are time limited
3. Maintains a working knowledge of theories and interventions that are utilized to successfully work with patients to provide empathy, support and encourage patients to initiate change.
4. Maintains a working knowledge of legal and ethical standards associated with the profession.
5. Develops and maintains clinical skills through seminars, workshops, or other forms of training/education; complies with all mandatory training requirements in line with the requirements set forth by the Board of Behavioral Sciences specific toward licensure requirements.
6. Actively engages and participants in supervision, informs the Behavioral Health Clinical Supervisor of all patient interactions, including any and all legal and ethical issues.
7. Provides counseling, crisis intervention and referral(s) to community based behavioral health services for patients demonstrating emotional, psychological, and psychiatric symptoms.
8. Provides and monitors referrals for support, educational, socialization and other resources to empower patients with knowledge and network systems.
9. Operates from an integrated care model stance as a member of the health care team and as the behavioral health expert at their respective site.
10. Services may be conducted either before, during or after a well-child visit.
11. Completes all session notes in the electronic health record within 5 business days.
12. Participates in interdisciplinary case conferences regarding patients care as appropriate.
13. Furnish listing of current and ongoing community resources for adjunctive services.
14. Maintains a working knowledge of NEVHC’s policies and procedures
1. Active registration as a Registered Associate Clinical Social Worker in the State of California per the legal requirements under the Board of Behavioral Sciences.
2. A minimum of 500 clinical hours of experience providing direct patient services in a medical environment, including working with adults, minors, adolescents, families in individual, group, and/or family therapy.
3. Demonstrate knowledge in Birth-5 work, attachment, assessing and work from a team-based approach.
4. Able to work a flexible schedule of 20 to 30 hours per week or full time as designated by position.
5. A working knowledge of business English, spelling, punctuation, and contemporary general office practices and procedures.
6. Must have excellent oral and written communication skills and be able to communicate in large or small groups and public speaking skills.
7. A working knowledge of theories and interventions that are utilized to successfully work with patients in order to provide empathy, support, and encourage patients to initiate change.
8. Typing/data entry skills sufficient to meet production requirements.
9. Effective verbal and written communication skills to communicate clearly and effectively with patients and others.
10. Effective work organization skills.
11. Ability to work effectively as a team player.
12. Ability to be flexible and work in a changing environment.
13. Sensitivity to the different cultures represented among members and staff.
14. Demonstrated ability to listen and communicate with others in a professional and caring manner including sensitivity with individuals from diverse cultures and lifestyles.
15. Demonstrated ability to set priorities for tasks to work effectively in spite of interruptions and under minimal supervision. Self-starter, reliable and dependable.
16. Demonstrated proficiency with the electronic health record database within three months of attending training session(s).
17. Fluency in English (speak, read and write).
18. Fluency in Spanish (speak, translate, read and write) is preferred.
19. Ability to solve problems and make routine recommendations.
20. Ability to maintain absolute confidentiality about health care and other patient/client information.
21. Current California driver’s license, appropriate insurance coverage and a driving record acceptable to the NEVHC’s insurance carrier (if required to drive on the job).
