Job Description

The Manager, Outreach and Engagement works with community-based organizations and healthcare agencies as well as health plan members and their caregivers to increase participation in Alzheimer’s Los Angeles’s healthcare client services programs, including Enhanced Care Management (ECM) and care navigation and support.

This is a hybrid position requiring some in-person work in our offices or in the community in/around Los Angeles County. Occasional weekend and evening work may be required.

Responsibilities:

Supports development of outreach and recruitment strategies that successfully engage appropriate participants.

Identifies, establishes, and maintains strong relationships with Community Based Adult Services (CBAS) sites, Dual Eligible Special Needs Plans (D-SNPs) healthcare providers, and other ECM and Community Supports providers.

Conducts outreach to identify, recruit, and enroll eligible participants into ECM and Care Navigation and Support programs. Conducts telephonic, in-person, and virtual outreach and offers in-service presentations in areas where members/caregivers live and seek care or find most easily accessible.

Fosters connections with families and energetically promotes engagement into identified programs by putting people first and effectively building trusting relationships.

Maintains accurate and detailed records of all outreach efforts, contacts, meetings, and program delivery.

Works with marketing teams to create outreach strategies and messaging materials to help promote identified programs and ensure materials are culturally and linguistically appropriate.

Creates and distributes health promotion education materials.
Provides recommendations and guidance, and requests information from healthcare and community service providers, as appropriate, to help increase participation into identified programs.

Participates in initial and ongoing staff training regarding common issues in neurodegenerative diseases, aging, caregiving, trauma-informed practices, person-centered care, motivational interviewing, and other topics, as assigned.

Participates in evaluation and improvement initiatives by contributing feedback during team meetings, surveys, and one-on-one meetings with supervisor.

Qualifications

Minimum Requirements

2-3 years experience in community field work
Excellent verbal and written communication and presentation skills
Excellent organizational skills
Ability to work effectively and collaboratively with interdisciplinary teams, program participants, caregivers, co-workers, and healthcare and community service providers
Excellent analytical and problem-solving skills
Ability to work effectively in a fast paced, team-based environment, under supervision and direction
Ability to prioritize and complete tasks in a timely manner
Understand importance of trauma-informed care and harm-reduction approaches
Ability to work with any member or family regardless of race, gender, sexual orientation, religious affiliation, cultural beliefs, lifestyle, disability, and disease process or treatment plan
Friendly, enthusiastic and positive communicator, committed to member engagement and retention
Computer literate -- Word, Excel, PowerPoint, Outlook
Bilingual in English and Spanish
Successful background screening
CA driver license, proof of auto insurance, reliable transportation

Preferred Qualifications

Masters degree preferred in public health, gerontology, social work, or related field
Knowledge of Los Angeles County aging community resources, networks, and programs
Experience with health equity and health literacy
Experience with data management systems
Trained in Motivational Interviewing
