Essential Job Functions:
Provide clients with basic information regarding the programs and services.
Assess patients for appropriateness of participation in treatment and level of care.
Administer ASI, ASAM, psychosocial and other diagnostic assessments under the supervision of the clinical manager;
Work collaboratively with Contract Coordinators, Substance Abuse Counselors, the Community
Services Manager and other staff in providing best practices, promising practices, emerging practices and outstanding care to clients;
Assist in developing a policy of integrated care, within the Outpatient and Community Services
Department, to the greatest extent possible;
Refer clients to other agencies, for appropriate coordinated care, and assist in developing of policy of coordinated care involving the Outpatient and Community Services Department and like-minded community partners;
Work collaboratively with members of the agency’s Clinical Management Team;
Charting and other required documentation, including data entry using SAGE
Facilitate or co-facilitate occasional psycho-educational groups;
Provide clinical supervision of substance abuse counselors, master level interns and group facilitators, as requested;
Assist with LPHA functions, finalizing medical necessity, clinical oversight & determinations and review intakes;
Provide individual counseling sessions with substance abuse, mental health and co-occurring clients, as scheduled;
Provide DSM Diagnoses, when requested;
Review and approve treatment plans, when required;
Comply with confidentiality and HIPPA requirements;
Comply with requirements regarding mandated reporting;
Comply with all agency practices, policies and procedures, including but not limited to CARF requirements;
Provide timely and professional responses to internal and external communications;
Participate in continuing education, as required by Licensing bodies and / or agency;
Schedule, coordinate, attend and participate in regular, on-going in-service trainings for the benefit of the agency;
Participate in staff meetings as required;
Further the Outpatient and Community Services Department’s commitment to client-centered, trauma informed, recovery orientated, culturally competent continuum of care;
Must review and be familiar with the agency’s EOC (Environment of Care) plan;
Other duties as requested by Clinical Manager or the Director of Outpatient and Community Services.
