Job Description

RESPONSIBILITIES:
- Conduct individual therapy sessions using evidence-based therapeutic approaches such as cognitive-behavioral therapy (CBT), motivational interviewing, and psychopharmacology.
- Makes mental health diagnoses as part of a treatment plan as needed.
- Conducts psychosocial assessment of patients upon admission, annually, and/or as clinically indicated to evaluate social, emotional, and physical needs by applying social work theories and techniques. Identifies and addresses barriers to care.
- Manages independently assigned caseload of varying clinical acuity.
- Provide comprehensive assessments and diagnoses for clients across the lifespan.
- Develop and implement treatment plans based on client needs and goals.
- Collaborate with multidisciplinary teams to coordinate care and ensure holistic treatment for clients.
- Maintain accurate and up-to-date medical records.
- Advocate for clients' rights and access to resources within the community.
- Provide crisis intervention services as needed.
- Experience working in Federally Qualified Health Centers (FQHC)/ Community Health Centers or Primary Care Outpatient Clinics; Medi-Cal, Medi-care patients preferred.
- Refers patients to appropriate community agencies, as needed.
- Prepares social case histories with particular emphasis upon psycho-social factors affecting the patient.
- Cooperates with staff and other community agencies in carrying out treatment plans.
- Evaluates new techniques for dealing with emotional and environmental problems.
- Performs other related duties, as required.

Qualifications

- Master's degree in Social Work (MSW) from an accredited program.
- Current California state licensure as a Clinical Social Worker (LCSW).
- Experience working in a clinical setting, preferably with a focus on Medicare and Medi-Cal populations.
- Excellent interpersonal skills and the ability to establish rapport with patients from diverse backgrounds.
- Strong assessment and diagnostic skills.
- Knowledge of behavioral health and the skills to assess mental health functioning, high risk behaviors, depression, anxiety, or other psychiatric conditions impacting hospitalization or transitions of care.
- Knowledge of substance use and the skills to assess level of addiction, motivation for change, and to develop and implement an appropriate plan of care.
- Knowledge and understanding of the influence of cultural and spiritual values in social work practice.
- Knowledge and skills necessary to provide psychosocial care appropriate to the age-specific needs.
- Proficient in using a computer to accurately enter and extract data, send and receive email, calendar appointments, and use task lists as will be using a variety of computer software programs.
