LCSW Case Manager

We are seeking a Clinical Consultant for our Enhanced Care Management (ECM) program. This position is part of our ECM team and is located in person at our Hawthorne and Los Angeles offices. The ECM program is an innovative initiative designed to support and empower high-risk, high-needs patients in improving their health outcomes.

Our multidisciplinary team, which includes registered nurses, lead care managers, therapists, and community health workers, collaborate to assist patients and address the social determinants that influence their health. The Clinical Consultant, who must hold an active LCSW license with the Board of Behavioral Sciences, will support the case management team through various activities. These activities include outreach and engagement, developing formal care plans, and linking patients to essential resources such as food, housing, transportation, and financial benefits.

Additionally, the Clinical Consultant will connect patients to medical and behavioral health care, both within our clinic and through external medical systems. This role will provide clinical oversight and consultation for case managers who work with complex individuals.

The administrative duties for this position will require 24 hours of work per week, spread over three days. Additional hours of direct patient care may also be available and are preferred to further support patients across the agency. This in-person Clinical Consultant position reports to the Hawthorne and Los Angeles offices.

Responsibilities

Conduct comprehensive patient assessments to develop individualized care plans.
Provide clinical counseling and behavioral therapy to support patients' mental health needs.
Maintain accurate medical documentation and adhere to HIPAA regulations.
Collaborate with multidisciplinary teams to enhance patient care and treatment outcomes.
Utilize crisis intervention techniques as necessary and engage in ongoing patient evaluation.
Offer support and guidance to families, particularly in child and family counseling scenarios.
