Clinical Social Worker III

As an integral part of our program, you’ll provide compassionate and comprehensive psychosocial care to patients, addressing their unique needs through counseling, case management, and expert referrals. Your role involves charting patient interactions, ensuring seamless communication and continuity of care within our advanced electronic medical record system. In addition to your direct patient care duties, you’ll be instrumental in updating and enhancing our program’s resource referral networks and websites, curating accessible information and resources tailored to our patients’ diverse needs. You will be essential for identifying patients and addressing their comprehensive psychosocial requirements, while also participating in the ongoing development of our program initiatives. With your LCSW degree and experience in medical environments, you bring invaluable expertise to our team. Your ability to navigate complex medical systems empowers patients to access essential services, ultimately improving their quality of life and that of their families. Required: * Master’s degree in social work and licensed in California * Knowledge of psychosocial and other rehabilitation relatedproblems that arise in individuals with complex conditions at all phases of their disease and treatments. * Knowledge of psychosocial distress screening instruments. Ability to formulate assessments of psychosocial needs of individuals with complex conditions and frequent hospitalizations. Ability to put these skills to use. * Skills and knowledge to deliver psychosocial interventions in a medical setting. * Skills required to facilitate individual, group and family therapy interventions. * Skill in completing and entering patient notes, tracking forms and consultation notes at appropriate intervals into database. * Ability to provide clinical knowledge to less experienced clinicians including developing didactic trainings, evaluating students, field instructor accreditation. * Ability to adapt to changing priorities including seeing patients at multiple sites, under various conditions, without a specific office.
