Clinical Social Worker (Health Facility)

Clinical social work in long term care. Facility includes the following levels of care: assisted living, skilled nursing and memory care. Social workers are members of the interdisciplinary treatment team with a psychosocial scope of practice. Social workers are included in the facility behavioral health team. Assessment, care planning, crisis intervention and inter-disciplinary collaboration are daily aspects of the job.
