Clinical Social Worker (Health Facility)

The full-time CSW will be responsible for a caseload on the skilled nursing unit. The CSW is responsible for monitoring the cognition, behavior, and mood of their residents, as well as assist with various psychosocial needs and case-management. The CSW works as part of the interdisciplinary team, along with medical, nursing, rehab, and other disciplines.

MSW required, LCSW preferred. If unlicensed, LCSW must be completed within 4 years of hire.
