The LCSW (Licensed Clinical Social Worker) is responsible for performing both direct care and administrative duties focused on the completion of psychosocial assessments, inclusive of educational and community assistance for patients.
The Licensed Clinical Social Worker (LCSW) is a licensed and highly skilled health care professional who is a member of the interdisciplinary patient care team. Under the direction of the Chief Medical Officer the LCSW provides initial behavioral health evaluations, psychotherapy, trauma informed care, stress management training, motivational interviewing, crisis management, and community referrals for patients demonstrating emotional, psychological and/or psychiatric symptoms.
Essential Duties & Responsibilities:
These services include, but are not limited to, the following:
· Provides initial behavioral health (BH) evaluations and ongoing established BH visits.
· Develops BH care plans for individuals with psychosocial needs and concerns.
· Develop diagnosis and treatment plans that are integrated with the medical treatment plan, if applicable.
· Strong knowledge of CA laws related to ethics, PHI, HIPAA.
· Coordinates treatment in collaboration with Psychiatry.
· Identifies and supervises case management needs for patients with complex needs.
· Refer patients to other appropriate resources for support and treatment.
· Collaborates with interdisciplinary care team members.
· Respond immediately to medical provider requests for a behavioral health intervention in the patient exam rooms.
· Provide therapeutic interventions when behavioral health issues arise during a medical clinic visit.
· Use behavioral health interventions as part of an integrated response to a patient’s medical and mental health needs.
· Coordinate long-term therapy for patients with chronic/severe mental illness.
· Maintains safe and clean working environment by complying with procedures, rules, and regulations.
· Works in collaboration with the Clinical Administrator, Clinical Operations/Compliance Director, and Quality department, and other clinical staff to develop, establish, optimize, and implement clinical protocols and procedures and quality assurance/improvement plans.
· Adheres to all SCHG policies and protocols and follows protocols for proper documentation in accordance with the quality and compliance department. This also includes timely completion of charting.
· Attends monthly provider meetings.
· Attends monthly QI/QA meetings as needed.
· Participates in quality improvement activities related to BH services, outcomes.
· Collaborates on revisions to processes, protocols, and policies related to BH services.
· Protects patients and employees by adhering to infection-control policies and protocols. Complies with federal, state, and local legal and professional requirements by studying existing and new legislation; anticipating future legislation; and enforcing adherence to requirements and advising management on needed actions.
· Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional societies.

License/Certification: LCSW (Required)
Work Location: In person
- Master’s in Social Work from accredited institution
- Active license as a LCSW by the California Board of Behavioral Sciences (BBS)
- A working knowledge of theories and interventions that are utilized to successfully work with patients in order to provide empathy, support, and encourage patients to initiate change.
- Demonstrated ability to listen and communicate with others in a professional and caring manner including sensitivity with individuals from diverse cultures and lifestyles.
- Expertise in evidence-based practices such as Solution-Focused Therapy, IMPACT model, SBIRT, Motivational Interviewing, and Harm Reduction.
- BLS certification
- National Provider Identifier (NPI)
- Bilingual English/Khmer preferred
- Able to lift/move up to 50+ lbs.
