Description & Purpose:
The overall goal of the ECM benefit is to provide comprehensive care and achieve better health outcomes for the highest-need beneficiaries in Medi-Cal. The Lead Care Manager (LCM) plays an integral part in providing and coordinating care, including assessing the client’s condition and providing them the necessary resources, options, and coordination for a healthy care outcome.

DUTIES AND RESPONSIBILITIES:
Coordinates the delivery of services from various social service providers within the community to the program participants.
Assists program participants with Probation and Parole requirements, such as providing progress reports and supportive documentation about the program.
Conducts individual needs assessments, disseminates information, and assists the participants in understanding social service protocols and procedures.
Supports and promotes open communication and cooperation among participants and leadership.
Ensures that Rooted Life confidentiality policies and procedures is adhered to and always followed.
Coordinates all medical appointments and keeps medical files updated
Maintains a positive, life-giving environment for the residents.
Keeps client files updated with medical, legal and social service-related documents.
Attends weekly team meetings.

Language Ability:

Computer Skills:
To perform this job successfully, an individual should have knowledge of Microsoft Word, Microsoft Excel, and Microsoft Outlook.
Education/Experience:
Bachelor’s degree in a related field with internship experience or three to four years related experience and/or training; or equivalent combination of education and experience.
Certificates and Licenses: Valid California driver's license
