The Job
The purpose of the Behavioral Health Specialist is to collaboratively assess, plan, implement, coordinate, monitor, and evaluate the member’s mental health needs through a collaborative care model. Focusing on members’ behavioral and mental health serves to holistically support the member and potentially prevent unwanted or unnecessary events across the continuum of care.
You Will
The Behavioral Health Specialist would function in the following ways
Provide short-term mental health treatment to members including assessment, diagnosis, treatment planning, interventions (individual and group), and discharge/aftercare planning.
Manage members identified with mild to severe mental illness.
Ensure and evaluate clinical appropriateness of care plan by incorporating assessment findings against evidence-based guidelines, clinical reasoning, clinical practice guidelines and/or best practices in the community.
Provide education and coaching surrounding behavioral and mental health therapies, interventions, and pharmacological approaches.
Create interventions and overall plan of care for members identified with a BH/MH diagnosis through risk stratification.
Increase the ability of clients to live safely in the community by working collaboratively with family, caregivers, primary care team, psychiatrist, community resource providers and others involved in the life and well-being of the client.
Enhance department and organization activities by identifying opportunities for improvement to existing processes.
Participate in activities to support professional growth through education opportunities, reading professional publications, maintain professional networks, and participating in professional organizations.
Maintain confidential information by following all HIPAA regulations, policy and procedures, compliance and regulatory directives and by completing required documentation in a timely, professional and accurate manner.
Actively support the achievement of SCAN’s vision, mission, and goals.

Your Qualifications
Master’s Degree, required.
LCSW or LMFT, required.
2-5+ years Behavioral and/or Mental Health Experience, required.
3+ Year post-Licensure Clinical Practice, preferred.
Blingual Spanish, preferred.

Motivational interviewing
Ability to build strong rapport
