Managed and operated by CSI Support & Development Services, a resident/member controlled organization that utilizes a cooperative management system and engages its resident membership in decision-making at every level of its operations. The Resident Service Coordinator will provide service coordination, assessment, case management, advocacy and linkages to seniors and adult residents with disabilities living at Magnolia Towers Apartments. The Service Coordinator will establish linkages with agencies and service providers in the community who serve seniors and people with disabilities. The Service Coordinator will work and consult with the resident cooperative running the building and will provide education to the coop regarding the needs and appropriate service delivery to this population. RESPONSIBILITIES • Provide intake, assessment and referral services for seniors and adults with disabilities; • Provide ongoing case management for those individuals who need this service in order to link with and utilize existing community resources; • Establish and maintain linkages with service providers in the community; • Provide or coordinate educational sessions for residents about the resources available to them within the broader community; • Monitor the ongoing services that are delivered to residents by community providers to assure that they are appropriate and that the resident is progressing; • Develop and implement individual service plans for each resident who receives ongoing case management; • Maintain accurate, timely and confidential documentation on all clients served; • Organize monthly educational workshops and community events for residents; • Assist residents to develop informal support networks with family, friends and other residents; • Educate the resident cooperative about the needs and services available for seniors; • Develop and maintain an onsite resource guide of service providers available to the residents; • Serve as an advocate for residents to ensure that they receiving all services for which they are eligible; • Work collaboratively with the quality assurance staff to assure that program outcome measures are met; • Provide monthly input e.g., statistics, number of clients served, center activities/classes to the Quality Assurance Supervisor, for timely completion of written reports. • Attend monthly quality assurance treatment team meetings to ensure that program outcome measures are met; • OTHER SKILLS AND QUALIFICATIONS • This position requires BA/BS in gerontology, social work, or equivalent social service experience with a minimum of five to eight years of experience in social service delivery preferably with a focus on senior citizens and people with disabilities or an MSW with a minimum of two years experience. • Bilingual required (Russian) with excellent oral and written communication skills. • Working knowledge of Federal, State, and local laws and requirements related to elderly and disabled programs, as well as non-governmental networks related to geriatric services, e.g. homemakers, Meals-on-Wheels, disability counseling, and local Agencies on Aging. • Understanding of the physical, mental, and social changes associated with elderly and disabled populations, including cognitive impairments, medication/substance abuse, fraud/violent crime awareness and self-protection, and dealing with death and loss. • Demonstrated working knowledge of supportive services for senior citizens and adults with disabilities available in the local community. • Demonstrated ability to advocate, organize, problem-solve, and obtain results for seniors; • Computer literacy in Microsoft Office and case-management software. • Completion of 36 related classroom or seminar training hours prior to hire or within the first twelve months as Services Coordinator. • The candidate should be capable of clearing a national criminal background check, and has at least five years of acceptable work history. ----------------------------------------------------------------------------------------------------------------- If you are interested in becoming part of the team, send letter of interest and resume via email to: damundson@mjhousingandservices.com Address: North Hollywood, CA 91601

The Director of Interim Housing is responsible for the oversight, direction, training and supervision of LAFH crisis housing for families. Duties include but are not limited to social services staffing oversight, providing case management and support services supervision, resolving operational issues, contract management including compliance reporting, maintaining and increasing collaborative partner relations, and community engagement and promotion. Assist with fiscal contract management – development of crisis housing program budgets and ensures submission of any needed documentation for cash requests.  Coordinate quality assurance reviews to ensure effective program operation, including file and facility inspections.  Select, supervise, evaluate and report on the professional development and job performance of all personnel responsible for the provision of supportive services and client programs.  Oversee the development, enhancement, and delivery of all crisis housing supportive services and programs.  Lead and supervise launch of new LAFH crisis housing locations including hiring staffing, managing operation budget, maintenance oversight, family placements, protocol and guidelines etc.  Identify and develop areas of improvement of staff, procedures, policies and building functions of Interim Housing.  Collaborate/participate in grant preparation/presentation and fund raising efforts to support crisis housing program operations.  Develop new relationships with area motels and service providers to build crisis housing stock. Communicate with others FSC’s to identify available beds in case of emergency.  Manage and maintain relationships with crisis housing partners including, but not limited to, Hope of the Valley, Ascensia, San Fernando Valley Rescue Mission, Family Promise, Hope Gardens and area motels. Coordinate with LAFH Volunteer Services Program to ensure optimal utilization of volunteer and donation resources in the effective crisis housing operation.  Coordinate with and oversee the LAFH Maintenance Department to ensure effective and efficient upkeep of the physical facilities. Provide oversight to and maintain positive relations with the neighborhood surrounding crisis housing programs – including both residential and community business neighbors.  Responsible for establishing departmental policies and procedures and department .
