A Case Manager: works within a multi-disciplinary team to assist in the development, implementation and monitoring of a service/treatment plan for each client that identifies the client’s personal, social, emotional, mental health, education, physical and/or financial needs and goals. Reviews and collects information related to the client’s socio-economic history, treatment history, educational history and other relevant data for the formulation of a distinct service plan. Participates with staff providing screening services to clients who attend the Clinic’s services, including completion of pay or financial information (PFI’s) forms and other needed/assigned documentation. Attends case conference meetings to assist and participate in case disposition and follow-up. Mediates and advocates on behalf of the client to help him/her obtain and keep suitable foster care/housing, health care, jobs and mental health services, education/school services, benefit entitlements, etc. Searches the community and presents options for additional needed or desired services (ei., emancipation services; sober living opportunities; education enrichment; scholarships, grants or job training programs; age-appropriate social opportunities; parenting classes). Performs other duties as assigned.
The position requires a BA/BS in social work or related field OR a license as an LVN or LPT, with an emphasis in community mental health work; a demonstrated history of presenting options for needed or desired services; a valid CA driver license and insurable driving record to make home, school, job site/or other community-based visits in own vehicle to assist the client in living, learning and working successfully. Bilingual/Biliterate Spanish ability highly preferred. In keeping with Clinic policy for all employees, the successful applicant must also complete a test for TB and fingerprint clearance.
